Jigsaw designed that one, Hoffman did not tamper with the trap's design or intentions, Jigsaw himself was on the tape explaining and demeaning the janitor for smoking. Hoffman was a garbage character anyway but the point wasn't to show Hoffman was targeting innocent people, it was just flat out shit writing.
Shit writing as in the Saw 4 - 7 writers didn't understand the Saw series to begin with. Their intention at that point was for Jigsaw to be a vigilante, you know, completely missing the point of the character, so in the context of "shit Saw", which Saw VI is, the janitor being in that trap makes no sense and is hilarious, but what'd you expect at that point since the series has devolved irredeemable trash so who cares anymore. In the context of the good Saw movies (the first few) Jigsaw putting someone in a trap for smoking is top tier and is exactly the kind of petty reason he would torture someone. He's angry that he has cancer and this person who's been smoking like a chimney his entire life doesn't. So I'm gonna put this piece of shit in a torture game because I'm spiteful and bitter. That's back when the series was great and Jigsaw was a scary villain. Saw VI is when the series was schlock garbage.
If you willfully kidnap people and force them in death trap dungeons where they must face severe bodily injuries or death and they die. Then you are responsible for those deaths. They would not be in that unique deadly situation without your direct interference
