Building bad habits / not studying enough. You really don't want to be playing blitz at your level, it should be Rapid games until you get more board experience, opening knowledge and better pattern recognition. You can think of your rating at 1200 as the sort of lucky variance that a new poker player might experience before coming back down to earth.
Yes. Lichess uses Glicko 2 which starts you at a higher rating. Chess.com and FIDE both use Glicko 1 so they're approximately closer but OTB is just a different game.

It's rare to not play a cheat at least once a session anymore. If you're going to take up the game I would recommend doing it purely traditionally as much of a pain as it is. Go to a chess club, sign up with FIDE, play OTB
