The movie is sex-positive throughout, including when she has the brain of a toddler. She doesn’t suffer any negative consequences like pregnancy or disease and she doesn’t regret it. Any character who has a problem with her being a prostitute is portrayed as silly and unreasonable. The characters we’re supposed to like all accept or at least don’t vigorously object to the little girl getting her brains fricked out on the reg.

Oh yeah like the time she ran away with an insane lawyer and ended up trapped at sea? Or when she gave away all the money to some sailors and ended up homeless in Paris? So she had to become a prostitute and live with a boss who'd randomly bite her?
She was happy to be a prostitute, it wasn't her money she gave away, she felt better having given it away and she had reserves on top of it. The only decisions she makes that harm her and she seems unsatisfied with are sticking with Ruffalo as long as she did and maybe going back with her husband.
She's the hero. Of course, she's going to take her suffering in stride.
She takes the suffering of being kidnapped and being back with her husband in stride, there just is no suffering at the brothel or in the early stage of her time with Ruffalo, and despite his kidnapping of her, she ultimately makes the most of her time on the ship anyway.

The biting is hardly suffering, the brothel owner is ultimately caring and she has a positive relationship with Bella. She doesn't like that she has to have sex with weird ugly dudes but makes the most of it anyway and it is, at its worst, just a slight unpleasantness for her and she finds amusement in the novelty of the different men. I've been seeing a lot of people saying the sex scenes in this aren't supposed to be glamorized or portrayed positively, but I don't get where they're coming from. She's never remorseful over it, not enjoying herself or suffering any consequences from it, the worst being that she doesn't want to sleep with every freak that walks into the brothel, which she can tolerate anyway. The only sexual moment that felt like it had a negative portrayal to me was Ruffalo coming onto her when she was blowing bubbles, seeing as the being enamored by blowing bubbles was a very infantile act so it felt like it was emphasizing her still being a child then.
I still did like the movie overall even though I didn't especially care for its theming, but the sex, even though I personally think it distracted from more interesting parts of the premise as it went on, felt like it was supposed to be an empowering and enlightening experience for her almost the whole way through.

I'm in two minds about it. On the one hand I really like how it's made, the sets, the score, how Mark Ruffalo's character ended up in an institution because of a moronic woman. But the whole sex thing with her was incredibly creepy, especially in the beginning when she's even less developed mentally.

That it's creepy doesn't bother me so much in itself, though I do find it odd how it feels both aware that she's still infantile when Ruffalo takes her away and still shows her getting fricked by him as a general awakening to the fullness of life in general, but I just think the sex-focus took away from the film in a broader sense because there's a lot more she could be experiencing about life in general but it just always goes back to sex. When I think of the more memorable parts of the film I think of her exploring Lisbon or the part in Alexandria or when her husband came back and her autonomy was being discussed in a way beyond just having her display it by having sex. I think her being obsessed with sex to start was fine because it's a base impulse and it made sense that with her reduced intelligence she would pursue hedonism, as she did with all the food she was eating too, but then she just kept on doing it while just dabbling in anything else like reading until she's called back to London in the end.
The focus on sex just got tired for me after a while and the glimpses of her interfacing with any other facet of life always felt like they were getting cut short to focus on her sexual autonomy again which I ended up finding more boring than creepy in the end. I also don't know if it was intentional, but it felt almost like a parody of middle-upper class white women with her going from whoring herself out as self-exploration, pouring her heart out for those in need and doing nothing about it than give money to others who won't use it to actually help those in need, spin the whoring as an act of intellectualism and then settle down and marry some guy who'll look past it when you're done having your fun.
