I am guessing 1 vs 1 and survival were included to make sure villages don't play favorites by raising a super soldier. Not because of the violence, or they were afraid I would try and imitate the stunts. And they let me watch even more violent anime like Inuyasha and FMA. Naruto not getting promoted isn't a plothole. It's stupid, but it doesn't invalidate anything in the plot. In fact, it holds up an established fact in the story that you can't rank up beyond genuine unless you pass the chunin exams, which he did at the end of the story and then they promoted him to hokage. It's bureaucratic nonsense and upholding tradition and protocol and yatta yatta yatta, it's actually one of the most realistic parts of Naruto.
