Yeah weird random shit happens in real life. All those stories from the world wars about people getting shot in the heart but their bible or a whiskey flask took the bullet...sometimes that genuinely did happen. Even the start of WWI was caused by an assassination attempt that's a complete frick up and they only pulled it off because of dumb luck.
the whole point of this scene is that it was a freak occurance that was miraculous that he didn't die.
he gets ptsd after it and literally goes to therapy and talks about how insane it was that he wasn't killed and it causes him to get a different perspective on life for a time. its literally about how bizarre and strange it is that he didn't die. thats the fricking point.
Handguns are surprisingly difficult to shoot accurately even at just a couple yards and these two thugs were moronic. it's not completely unbelievable.
The first fella noticed that the juice said "with pulp" and not "with some pulp." Being aware of Tony's pulp proclivities, he tried to save him from a nasty surprise. The second fellow was simply trying to protect his friend from a fatal misunderstanding.
It's all being misinterpreted to make Tony look the victim, as this show tends to do.
