My take when I first watched it was that he was gaslit into thinking he was crazy during that interview with Ben Kingsley's character. I appreciate that half the film makes no sense that way but I don't care.
It's a way better film if teddy was right and his wife died in a fire, also explains why Scorsese made the twist so obvious it's misdirect

For example, one woman's husband constantly kicked her ass and abused her, so when she snapped and stabbed him, she was labeled as "crazy".
The staff was extremely abusive and made normal patients crazy and crazy patients worse

These sort of mental illnesses don't really exist the way movies show them. His delusion would be forced by himself, but ultimately he would still know. So yeah he was pretending to be crazy, but that's crazy. So he's crazy.

It's a little more complicated than either "he's pretending" or "he's crazy". He's probably moved towards lucidity several times and had periods of immense guilt between his escape fantasies. The movie implies that this is the closest he's ever come to accepting what he did and then it slips from him at the very end.
He chooses to accept the fantasy at the end. I wouldn't quite call it "pretending". He's basically in extreme denial, to the point where he believes himself to be another person entirely.
He for sure is 100% pretending to be moronic at the end because he wants to die.
Shit adaptation from a fantastic book. First problem it's Leo, he is not the right cast. In the book the guy is HUGE, imagine The Rock or that guy from the Reacher show. So everyone is afraid of him. Also there is more nuance and fricked shit, we never know if it is him, his wife or both that are crazy. There is implication she is either cheating on him or being molested by people around town, we don't even know if she really killed his kids or him killed his whole family. Thing is, he lost after the war and is suffering from PTSD, becomes more and more violent, wife is suffering from depression and maybe is bipolar, one day he comes home and says she killed the kids and herself, cops thinks he did it. Also he has a lot of dirty since he's been beating criminals to a pulp.
In the end he is "cured", for a moment, and pretends to be unwell again to be lobotomized. Why? First, atonement, he did a lot of shit. Second, he don't trust himself anymore. The treatment worked, he has peace of mind, but decides that the world is a better place without him.
Coming back to the casting, it's difficult to accept all that since is fricking Leo. We can't picture him being that dangerous to the point that is better to get a lobotomy than keep on living, if he gets angry you just slap him. You need to sell the guy as this unstoppable monster, someone that will hurt you if he wants, a real danger to society.
But then Ezra Miller exists so maybe I am totally wrong.

I dislike twist movies/stories. Feels cheap. There could've been something interesting with missing patients.
Stop seeing it as a big twist and that being the only point of the movie. The point of the movie is you're watching from the patient's point of view his delusions.
