          I’ve watched the show six times over now, and I honestly never considered Butchie. He seemed content with their agreement to turn down the noise by letting Tony’s crew kill Phil. I do however think Patsy was in on it. He never got over his brother’s murder, and the fact that he got away unscathed seemed damn near impossible unless no one was aiming at him at all. It stands to reason he and Paulie would be the choices for promotion after Sil, Bobby and Tony were got.

      I think this is essentially confirmed, Chase said the original idea was for Tony to get lured into a meeting with NY at which, they plan to whack him and the episode would end with Tone on the train to NYC, only to cut to black as the train passes into a tunnel. He said that this was "basically the same" as what they ended up going with. The only reason it's a diner instead of a train is because Chase saw an American diner one day and arbitrarily decided that "this is the kind of place where Tony should die".

      Sopranos leans heavily on implication without explaining, just like the mafia. When Carmine Sr. called Tony to put a hit on Johnny Sack, he did it without coming anywhere close to explicitly saying it, but everyone still understood. David Chase liked to leave things open ended, like the Russian in Pine Barrens or if Jimmy Altieri was actually a rat. It’s a perfect ending for what the series was.

          Tony makes mistakes sometimes, like the stolen airline tickets. If the actress who played Livia hadn't died unexpectedly, it would have led to a major story arc where Tone is closer then ever to being convicted, all because of one stupid mistake.

          Tony was lucky often, one of the major rats dies unexpectedly, the wire in his own house is neutralised accidentally by his daughter, his mom dies before she can take the stand against him. He's a completely fallible and human character, he had to be lucky to last as long as he did and his luck running out is an appropriate ending for his story.

          He was really lucky, but I think the coma actually caused brain damage. Notice how impulsive and angry he was in the late season.

        Patsy would have been the new head, he was the most senior competent leader left in Jersey. He still owed Tony for killing his brother, and with meadow dating his son he would have been able to spy on the family to make sure no revenge was going to catch him unawares. He might have even taken on AJ as a driver like Tony did to Chris if AJ went off the rails again, which he probably would have.
