The police still had not caught the actual murderer that the story was based on when the movie came out, so he was looking at the murderer through the screen. I think they did catch him eventually though.
The murders were based off a real-life string of killings by someone who was never caught and the intention was that the character looking into the camera at the end would’ve been him staring at the killer as he watched the film.
The problem is, the killer was arrested for another murder before the movie came out and was only proved to have done the other killings years later, so he probably never even saw the movie.

Also, Oldboy was the last movie in a (thematic) trilogy, preceded by Lady Vengeance and Sympathy For Mr. Vengeance all listed in backwards order. I just finished Sympathy, and it was a solid 7/10. Not quite as good as Oldboy though.

It was actually a blooper. Song Kang-ho forgot his lines and looked right at the camera in despair. Bong thought it was poignant and fit the ambiguous tone of the movie, so it stayed in.
