There isn't a definitive answer for whether Tony dies or not - it's not important. If he didn't die that night, we know he likely will at some point. And he's waiting for it.

The thing is that Chase wanted to portrait is that it can happen at any time by anyone. But you don't see it coming. You didn't see it coming and neither did Tony. There's no Hollywood style shootouts or monologuing, you just get shot in the back of the head by your best mate or in this case (and a few others) you're out eating lunch and some random guy walks in a shoots you a few times without hesitating. No glorious deaths. You should have noticed that watching the series.

The entire series starts with Tony having a panic attack and reluctantly seeking out a therapist to help him. I believe the ending is supposed to imply two endings:
1. It's a bad ending and just like Tony feared, someone really did come for him.
2. It's a bittersweet ending. Tony might have fooled the system, but he can't escape himself. The very problem that started our journey with Tony remains a problem in the very last scene of the series, only now we know why. Tony is doomed to live a life where he and his family could die at any moment, and his panic attacks will never go away due to the anxiety this life causes him.
Either way, we get a very karmatic ending and I really do like it. Great way to end a great series.
