True yeah. But Billy also talked about wanting to marry her like 3 different times, and while Mandy is cruel to Billy, he's the only one she follows around like a puppy.
Her hanging out with him is the exception to her usual behavior. She genuinely doesn't give anyone else the time of day except him or Grim, and he's the only friend she has that isn't trying to frick her or wants to kill her or is afraid of her.
That's because they're, like, 9 years old.
The idea is a cross between how Dale and Nancy Gribble's relationship works in KotH (Dale's antics take up so much of Nancy's time that she doesn't have the energy to be an evil mega-b***h to people) and Billy being one of the few people that Mandy genuinely care about because he neither cowers nor caters to her meanness.

Billy and Billy's dad to an extent both suffered from stupidity Flanderization like Homer in the Simpsons. I personally thought Billy was hilarious but I can see how writing the show would be annoying because Billy's antics could constrain your plots.
Because you can't easily turn him into a cute anime boy and his existence keeps Mandy from fricking cool super-powered dudes.
