Various native tribes, mexicans, and settlers are graphically killed constantly throughout the book (stuff like scalping, impalement, raped to death, severed limbs, entrails pulled out). Dead bodies are paraded through the streets
at some point the judge finds a little boy in the aftermath of some bloody battle and adopts him for a bit, whole crew bonds with him and then one night out of nowhere the judge rapes the kid (like 5-8yrs old) up the ass and then snaps his neck like a twig for no apparent reason
the MC is raped to death in an outhouse by the judge
that's just what I can remember off the top of my head, the kill count in the book is probably in the hundreds if not thousands and many of the deaths are described in gruesome detail

Bone Tomahawk had that fresh yet classic feel I love in a western. If you removed like 2 minutes of footage it basically could have been a solid 1960's Western. But it took a frick ton of effort and talented people basically doing it as a passion project to get it done.

Plenty of talented guys out there with some leisure time. I've always thought that the form of a Western provides structure like in classical music or other types of stories, hero stories, tragedies, anything. It's more about finding something worth saying when you make a Western, even if it's a bit redundant. I'll watch old westerns and it will be something generic as frick thematically like Shane and still be top tier kino, because they stuck to the form and got creative within it.
It's really guys like Peckinpah who pushed the limits, but eventually you just start traveling up your own ass after a certain point, something gets lost in translation (see Heaven's Gate, a million other flops). But then once it's written, getting it off the ground is where most ran into a problem if you need any kind of money for it. Zahler got around this just by doing it all himself for Bone Tomahawk.
Cormac is the most overrated writer of the last century. I’d rather read Stephen kings entire works than slog through a punctuation-free pretentious Cormac novel

I don't think it is unadaptable, rather it is just much more enjoyable read than in film.
