        They used one of the analog sticks to move move forward and backward, and they used the other to move side to side. The playstation controller layout was probably less awkward for that since the sticks are both on the bottom. Maybe they wanted that design but needed to use xinput.

          Hand tightening is the correct thing, they user a torque wrench for final torque in the video. It avoids over tightening and stretching the bolts. However, steel can deform or become brittle at those pressures and it wouldn't surprise me to know they were using the cheapest Chinese "steel" fasteners

          Here is the kicker, everyone knew it was a matter of time till it failed. Because that is how CF works. So after every pressure cycle, you are supposed to inspect the CF for signs of failure. However since they used 5 inch thick CF, this made visual inspection impossible.
          So oceangate installed an audio device that listen for signs of failure in the CF. However again, anyone who worked with CF knows that if the CF starts creaking, it is going to fail in a matter of minutes. And based on reports that are now being released, that was probably what happened. It was releasing ballast trying to ascend faster so while the death was instance, prior to the hull cracking, the crew probably heard the alarm and were trying to surface as soon as possible.
