The characters in the novel meet him, learn he's the one writing their "story", and one of the characters sacrifices themselves to save him. In this telling, he would have been fatally wounded by the truck if the character had not pushed him out of the way.
King is one crazy frick but I think he's a pretty cool guy.

King was building on Heinlein's idea of "World-as-myth", that is, that the act of writing a story makes it real. The more people that know the story, the stronger and more detailed the characters in it become, as they are fleshed out by the collective imaginations of all the readers.
As the author, King was the world-builder and essential to Roland Deschain's quest to reach the Dark Tower. In order to ensure the quest is successful, the characters make safeguarding his life a priority over their own.

I've never understood the whole deal with periods being taboo or shameful. So many terrible female behaviors have been normalized and accepted without much blowback, so I don't get why discussing something normal and involuntary like a period has become a hot button issue.
