Ghouls can sustain themselves with radiation but too much will rot their brains. In the show they take FEV to stave it off, which is neat because FEV caused ghoulification originally
It was covered up 200 years ago to sell more suits to the US Army. The current BoS aren't skilled enough to spot minor issues in one bit of welding or have spotted it but can't just build an entirely new suit from scratch so are stuck with it and just hope they don't happen to run into a guy who wore one 200 years ago and knows about the flaw.
How come despite using the armor for 200 years the BoS knights aren't as skilled or knowledgeable as some guy who spent one tour of duty in the thing hundreds of years ago? How come in 200 years of use no one else ever thought to shoot the chestplate before discovering this flaw organically? How come in 200 years of maintenance no scribe ever had to fix this weld? If the weld was so flawed how did it last 200 years?
Soldiers in power armor before the bombs dropped would have either only trained in them, or been in full fledged major battles. Getting shot at from all sides.
How often does the brotherhood actually have major battles against well armed opponents who aren’t scattering? Not very often. That kind of experience is rare as frick for them
Plus these things are typically not put in there for no reason. They probably did try welding a few shut there and discovered the use overheats quickly and it was an important vent or some shit

he couldve shot and killed maximus with the amount of chances he was given in the first episode if he already knew a weak point in the power armor but for some reason he didn't
instead he kept firing around the weak point like a dumbass even though he ACCURATELY killed 6/7 or so men before max entered the scene

Ironically maybe he let him live so he could track him since he was also looking for the head. Which is what the squire suggests must be the reason he “let the ghoul live” to maximus
He wasted all his explosive revolver rounds on common wastelanders before Maximus showed up and couldn't exploit the weakness after swapping to his wimpy lever action, opting to ammo dump instead.
He purchased or found extra AP ammo in the interim prior to the webm encounter and even makes a point of showing it off for the camera.
I'm pretty sure he never intended to kill Maximus, just like Lucy - keeping them alive meant that he always had multiple leads to get him to the head/Lucy's dad. He says this at the end, that he'd rather keep the dad alive so he can lead him to his next target.
