superboy prime killing Pantha, definitely pushed him one more step over the edge. He truly didn't mean to kill there but at the same time, when you're a kryptonian and you throw blind punches any damage/injury you cause is 100% on you, intentional or no.

Goliath's death was sad, not because of the pointlessness of it, but because it was the moment when Tony should have backed down. The concerned soccer mom basically spits on his corpse and compares him to a gangbanger and then tries to make him forget that he killed one of his friends by shaking her dead son's Iron Man action figure at him. I'm assuming she never acknowledged how her spinning her personal tragedy out for political favor basically endangered the entire planet several times.
Daredevil accidentally killed a guy a during a fight a couple of years ago. He went to jail for it and basically refused most attempts to help him get out of it because of Catholic guilt.

In Terry's case some of those were either results of him wanting not to die or the villain chewing off more than he could process or being in the wrong place at the wrong time (or both).
Then again he did try to desperately save some of them like Peek or Mr. Freeze. Especially the latter. Some he did manage though.
I would argue that Terry, more than accidentally killing, just didn't have the drive to save many of the villains.
Alot of his villains weren't mentally ill like batmans, just cruel and deliberately evil
