Yes it does, in the Director's Cut when Mills asks John Doe "What's in the box" the killer reveals the actual contents: John Doe had actual cut a hole in the box and stuck his penis into it. When Mills opens the box he sees Doe's dick with a smiley face sharpied on the tip. Disgusted, Mills shoots John Doe and Somerset is forced to arrest Mills.

I’m not sure I’d even call it a twist, just something surprising that happens at the end.
The son inexplicably goes into a coma because of spooky shenanigans. The grandma knows a psychic to call because the father also had trouble with spooky shenanigans as a kid, particularly this spooky lady who was coming after him. Father has to open up to all this and enter the spooky realm to save his son, which he does... except the spooky lady from his childhood possesses him and makes him murder the psychic. The end
not a twist, but one of the best endings of all time.

The Mist ending is the one case where I prefer King’s version to an adaptation. And ironically, it’s the one King praised the most.
