Vault 12 was designed not to close to study the result of radiation poisoning on its inhabitants, which turned those who didn't die into ghouls, and many of those ghouls then left to establish some of the first ghoul-friendly settlements after the collapse of necropolis, but that was an extremely small minority of ghouls compared to the ones that already existed elsewhere and were constantly being created. Saying that ghouls originated from a vault is like saying that cars originate from a factory in a city in california. Not only were the vault 12 inhabitants not the only ghouls, they were not even the first ghouls.

The zany humour of FO2 is vastly overstated. In-game, it was a sparse scattering of throwaway gags (of which you'd only see a few on a given playthrough) in an otherwise fairly dramatic story. The ability to now just look them all up, completely removed from the framework of the game, had distorted the image of the game in hindsight
