Twist appears to be that the person that initially led them to the shelter was a watcher all along. They're studying people to better infiltrate human society so it's all been a massive setup.
The watchers are able to mimic people, the old woman is secretly a watcher all along.
They're just some Irish folklore trash in the universe of the book.
