She eventually got her memories back and they travelled to their home, but the humans weren't ready to explore the outside world yet. By the end of the series, they've had a change of heart and arrive at Ooo.
We don't know exactly when Jake died, and I can't remember if it was revealed how he died.

People forget that when the multiverse was first introduced in Adventure Time, it's literally stated by the Booko that the whole thing is shaped by people's consciousness. Like Prismo the wish master is an entity that exists as a manifestation of someone's dream.
Golb's connection is that he's the antithesis to that, he consumes universes and thus people's dreams as seen in Puhoy. It's also probably why ancient primordials like The Lich seem to deify or follow him, they seek to annihilate all of life or just destroy in general and Golb embodies that.
