Annie looking at Kat through the ether didn't see a huge, rusty, spiky machine angel, either, but Zimmy didn' t create it. Annie saw it independently of her through the arrow.
What Zimmy sees seems to simply be yet another layer of reality.
I guess? Maybe there is some kind of rule for whether Kat looks like The Shrike or not, but I've always had the impression it's just whatever the plot requires.
Considering that creatures from Zimmy's world can use her to get into the material world (the White Legs), but they can't do that through the ether, which a lot of people have the ability to see into, affect and enter, I think that it's reasonable to conclude ZimmyWorld is real and that it is part of a set of layers/planes/dimensions/whatever you wanna call it that make up reality.
It seems that what Zimmy has managed to do here is that she brought all three of these planes together, so Annie is seeing the new people differently in the ether because Zimmy's plane has combined with it, as well as the material world. This is probably a big issue, not least because of the aforementioned white legs that have been wanting to escape from their plane. Who knows what they are capable of, maybe they are chill as heck, but it seems more likely they are gonna want to invade the material world, pacific rim style.
