The whole troony thing was just to make the boy go through something as horrible and degrading as what the girl suffered. It wasn't meant to be seen as positive at all.
I assume he means when the setting becomes more overtly modern and realistic. I recall there being hints of it earlier like when the boy sees that construction work damming up the water but it becomes prominent after the escape. An interpretation I read way back then was that the twist was to show that the messed up archaic shit you see throughout continued all the way to the present day.
