    In the scene you can see Anton notice the flushing sounds and realizes he would need to off two people and so it's not worth it. The guys "moral code" or whatever does not trump his ability to do his job. The only liberty he takes is in going after the wife after he got the money back. Even then you could argue he was tying up a potential lose end.
    This fat slag did not have super powers, she was just lucky.

    When she affirms "I can't say" the first time, it's done with such a stern motherly tone that Anton is caught off guard and begins to devolve into a bratty, childlike state. Her nurturing yet unwavering manner reminds him that even with a deadly weapon in hand and a completely nihilistic worldview, the mere gaze of a postpartum old bat is still enough to completely shatter his composure. So he spirals. In fact, the rest of the film could be viewed as Anton rebelling against this old woman. The chiasmatic nature is mirrored at the end when he kills Carla Jean Moss, while the act was kept ambiguous to the viewer. It presents the notion of how much stock femininity can clash with an amoral ideology to turn men into caricatures of themselves. Though I might be looking too into it...

    She's wearing a fat suit as a disguise got these situations. Sugar returned later that day but because he saw only an innocent thin woman he left in frustration, which caused him to be sloppy in his fight scene against moss.

    Also I think the toilet flushing is meant to accentuate how easy it is to break Anton's illusory dominance of any situation he forces himself into. He's like a gritty cartoon character in a relatively grounded, serious world, so when similarly comic elements are introduced he acutely recognizes that dissonance and escapes the situation.
