I think it's mainly prison-based. It would seem that they're pretty darn big inside prison, perhaps the biggest. John Gotti had to look for protection with them in order to not get whacked himself. If the book "Blood In, Blood Out" is to be trusted (and I'm not too sure about that), they're perhaps the most dangerous since for 1) they have very good communication between even supermax facilities, 2) they have a robust hierarchy, so e.g., the top Commission was ordering hits on people from different prisons and 3) they have at least some outside influence, such as when doing drug deals inside the prison, the payment actually occurs outside. A trusted bud of yours will deliver the money and it'll get picked up by some NLR guys, at least that's how Silverstein supposedly did it. and 4) their goal is to be the most powerful organization in the world by being the most powerful in prison, because once some major leaders of other organized crime get booked in, they're at the mercy of the AB, e.g., Gotti as mentioned before, and usually major crime org leaders based in the US will get put in jail eventually.
