      Van Gogh was already mad, and it was likely due to whatever it was that caused him to see everything in such vibrant colors. That and the invisible alien that only he could see.
      It didn't help that his brain was so addled and wine soaked from drinking all of the time. Dude was a self defeating mess of self medicating with alcohol to come to terms with the fact that he could see "imaginary colors and creatures" that even the Doctor couldn't see.

              I think he knew from earlier because of the "right-handed don't hold with their left" comment; so he probably saw a bunch of inconsistencies about Vincent but kept quiet the whole time.
              I always thought that scene was real humbling since Vincent thought he did it all by himself, with his own sheer willpower, and he realizes at that moment that he had a lot of help from others that wanted to see him succeed like the Doctor, Jerome, and Cassini. That's what always gets me emotionally each time I watch that scene; that realization of humility when other people have been pulling for you that you didn't realize. My favorite scene.

      This happened to me as a teenager, except I didn't have much empathy at all and I could be quite nasty to other kids. I used to get 'excited' over creatures (animals or humans) being in distress. I don't know how I'd describe it other than it feeling similar to the butterflies rising up your chest sensation you get when you talk to your crush, it's pleasurable but in a perverse kind of way. One day it was like a switch was flipped and it became the polar opposite, now I'm overly empathetic to the point that it's debilitating, it's almost like I feel physical pain in my chest when I sense any sign of distress in other creatures. It sounds so dumb but it was like I downloaded a software update which fundamentally altered my perception of the world drastically overnight. I'll never understand what the frick that's all about

        I understand this to an extent. I used to have a morbid joy from watching shit like animal programs and seeing the baby animals get abandoned by its parents of hunted by other animals. Just sick shit so I made sure to avoid those triggers but nowadays I tear up just thinking about a poor little wildebeest or baby bird.
