He was literally responsible for every problem in the series. He ended up being worse than every Uchiha aside from Madara (who he both enabled AND saved).
Tobirama being selected as the next hokage rather than the next hokage being nominated was what caused it. The uchiha(probably) wouldnt have cared if they didnt get madara or someone else as hokage if they had a fair shake at it, but instead tobirama was hand picked. The police force stuff was salt in the wounds
The Uchiha believed themselves the rightful rulers of Kirihagahura and only begrudgingly accepted being tard-wrangled by Hashirama. They were never going to be accepted as Hokage because the other clans knew exactly what their opinion of them was, and putting it to a vote so they could tell them to pound sand would result in a temper-tantrum that would tear the village apart. (Hell, I think it's even mentioned that even the magical fairyland option of the Uchiha fricking off peacefully would result in the village getting torn apart as the others took the opportunity to invade.) Making them the special police (basically, sanctioning the shit they were going to get up to anyway) kept as much of the piss outside the tent as possible until a final solution to the Uchiha question could be found...which, to be fair, Danzo (PBUH) almost did. Tobirama's only mistake was in underestimating the degree of their Uchiha-ness, probably due to marrying one of the few sane ones.
It's more that they're supposed to support them so they can surpass them, which is why the problems are caused by those that refuse to let go of power (e.g. Danzo), those that refuse to take responsibility (e.g. the Sannin) and those that fail to think about the transition (e.g. looking in your direction, Hashirama, you little shit).

He nearly destroyed Konoha by alienating the Uchiha. Making them the police force was a good thing on the surface, but the villagers resented them for it. The Uchiha only started to plan the coup after the village moved them to the outskirts because of Obito controlling the Kyubi, crippling their resources.
In the third ninja war, Hiruzen made the Uchiha fight Kirigakure, a village that specializes in water release. Uchiha specialize in fire release. See where I'm going with this? Not to mention that the Uchiha got no credit for the war and Fugaku was never considered a serious candidate for Hokage after his merits in the eastern front.
They literally abandoned Madara and chose to stay in the village and got treated like shit for it. It's a surprise they didn't try to revolt sooner. Frick Itachi and frick Shisui, two worst characters in the entire series by far.
