The only explanation for how New York found Tony is that Carmella told them (she goes in, saying she's going to ask where everyone wants to go for dinner, then quickly comes out despite having to call Meadow and AJ, meaning she knew where they were going ahead of time when nobody else knew where Tony was) or Meadow told her boyfriend (Patsy's son) and Patsy was working with New York and put in the hit.
Unlikely that Paulie was involved, highly likely that Patsy at least signed off on everything and was made the new boss. What's most likely is that Carmella finally had enough and the last fight was the straw that broke the camels back.
