Isn't his name in one of the opening scrolls, either Attack of the Clones or Revenge of the Sith, he's definitely referred to as Master Windu several times

Jackson is credited as Mace Windu but during the actual movies they never say his name "Mace", just master Windu

How did we know his first name is Mace?

Yoda calls him Mace in AOTC
        "Help Obi-Wan you must, Mace"

The first time his full name is said is him introducing himself to Anakin, yes. As one anon said, most characters go by titles. I don't think Obi-Wan ever calls him anything other than "master" in TPM.

Fun fact: he got his first name because George Lucas was a big fan of the rapper “Ma$e” at the time. It’s ironic too since the rapper was part of a record label that George Lucas’s first pick, Tupac, was beefing with.

The name "Sheev Palpatine" is never said in any of the movies either. Star Wars tends to call people by their honorary titles because they are all weird alien people who act like stage play performers.

A lot of characters are never referred to by their full names in the movies. The Jedi, Politicians, and Military all refer to each other by honorifics.
    >General Kenobi
    >Master Windu
    >Senator Palpatine
    >Queen Amidala
    >Princess Organa
    >Grand Moff Tarkin
    etc. etc.

They only refer to each other on first name basis if they are friends or of a lower class.

Actually, I don't think the name "Organa" was said once during the OT. She's been "Princess Leia" since the opening crawl.

Still the point stands that there are many movies where the full name of a character may never be said.

It was in Sam Jackson’s contract that the lightsaber had to be purple, unironically.

It wasn't in the contract he asked Lucas. There's even a clip of him asking him in costume which means he already started filming by the time he asked.
