It's an alternative reality where police doesn't exist and people are apathetic by random violence as long as it doesn't put them in danger. Most of the characters are like npcs with a bad AI and John Wick levels up and get new perks through the movies while the difficulty of the game also increase.
You jest but it is basically a video game movie. It's like someone just decided to show the video game industry that movies are better at being video games then games are at being movies. The John Wick movies all function by pure video game logic, though, down to the doctor giving him pills to heal his wounds

The narrative implication is that the assassins and the high table have so much sway and represent so much of the civil body that these kinds of events are generally seen by the public anyways, and as long as their lives aren't in danger, they usually either ignore these things or simply move around them. The park scene at the end of 2 cemented that simple members of lower elements of the High Table's hierarchy are capable of manipulating crowds of people in the dozens (possibly hundreds) such that a simple phone call can cause them all to immediately jump to action.
The real issue is that this insinuates that the High Table is of such obscene prevalence in global society that their actions shouldn't be considered underground, and it's bizarre to have them operate in these pseudo-covert ways to mask their "criminal" operations. Without a single police officer seeming to even mildly attempt to stop anything going on, none of their enterprises ultimately make sense, as the prominence of crime is fundamentally tied to inaccessibility to certain resources or privileges by the public. Why would I need to contact some shady dude on a sidewalk for crack when I can simply figure out where my local High Table affiliate presides and get my crack from them? Why would I need to worry about hiring an assassin for some obscene price when I can just stab the fricker and face no retribution?
JW franchise is basically like a modern take of the wuxia genre. In wuxia, there's often the outlaw organizations away from the imperial rule/ royal court's rule. The outlaws have their own rules and influence and while there may be clashes with the government, the gov tend to turn a blind a eye, taking a 'you stay in your own lane, don't mess with our citizens, we tell our guards to leave you be'. There's even the hobo network run by Laurence Fishburne's Bowery King that parallels the Beggar sect that's a common element in wuxia. Hell, they neve name dropped wuxia as the radio broadcast in the finale chase.

Another trope is how often characters just randomly point guns at each other. In Part 3 Winston does an entire monologue gesturing to John while holding a gun as if to offer it to him (he's giving him a choice) but he's really just pointing a loaded firearm at him. Then, a few scenes later, John Wick just turns to Winston and points a loaded shotgun at him for no reason. lol

The movies take place in an alternate dimension where criminal organizations secretly run everything on top of that the people know they're at a nightclub owned by a criminal so they see people getting in fights all the time and don't care until they're directly in harm's way.

The scene where he greets the door man and tells him to take the night off carries a million times more weight and says more about these characters and their relationships then the 1000 goons he wrecks in the next 3 films combined.
The 3 sequels are almost a different universe of movies, though. There's no reason for any of these characters to trust each other or have the kinds of professional courtesy they showed in the first film. Ms. Perkins character got what was coming to her precisely because she abused that trust. In the films after it, it was just normal.
