diversity isnt contained within a show, its cumulative of all media. so the idea is to increase the total proportion of nonwhite characters present in everything.
Those two examples, while having a Europe-based setting, canonically have distant regions representing the rest of the world. GoT has canon non-white characters, and the Witcher has the area called Beyond the Seas which is known to be based on the Middle East and Asia, and likely more continents reflecting our own world.
Avatar, on the otherhand, has its entire planet mapped out. There's no secret fifth nation that's mentioned, just the four. There is one culture that depicts Caucasoids, however.

Yeah, they come from a specific place and a specific culture. It made sense. It was southern, hot and tropical. The one major black character in the books was Jalabhar Xho, an exiled prince from the summer islands. They cut him from the show

Well, there's Missandei and others. Race is kinda weird and vague in those books tbh - for example, Arianne Martell's parents are white, but many of her descriptions make her sound like a lighter shade of Ethiopian black.
The Velaryons being black is funny though. It makes a little sense, seeing as the Valyrians were a big empire that brought a lot of people into the fold in their middle period, but the black skin / white hair look is just off.
In the same way Zuko was meant to be Indian.
