I'm still working on the Silmarillion but it was always sort of unclear to me exactly what orcs/goblins are. Simply kidnapped and mutated elves? If they can breed with people and other orcs that must mean they are always breeding to create the orc armies? Then where are the orc ladies at?

It's never made explicitly clear how orcs come about, we only know that during the First Age, Morgoth created the first orcs by corrupting elves. Afterwards, through some dark magic, he released more of them into the world like a plague. It's sort of implied that Sauron breeds them through a similar dark magic, but the process is still unknown.

I think uruk hai refers to uruks from isengard specifically. Mordor has uruks as well and they were shown in the movies.

Isengard Uruks were technically still babies, with more experience they would been unstoppable
