When Arya gets stabbed by the Waif, actually let her die. Plant the seeds for Daenerys’ mind snap as early as when she meets Jon Snow in season 7. Let the Night King win the Winterfell battle in season 8 with minimal survivors + Jorah dies so Dany has no mentor + The north / Sansa openly distrust Dany because she couldn’t stop the Night King from destroying Winterfell. Dany’s depleted forces and bleak hopes for survival because of the White Walker onslaught influence her to torch King’s Landing out of spite and mental illness. Night King dies in the process but also the population of Kings Landing because of Dany. Jon kills her and either becomes king or they discard the political system of Westeros.
Jon kills her, then marries Cersei, because it is the only way to keep the realm together after all the bloodshed. They hate each other, but (unlike the peasants) they know that there will be another Night King at some point and they need to heal the realm and enforce the Wall.
Everyone starts with big dreams, but realpolitik gets them all.

This is the first moment they made it clear that some characters do actually have plot armor, and in the story that got popular on the notion of fricking up actually having consequences, it was damning for the future of the show. There were moments before this that showed a bad trend, but this was the one where it was obvious that they weren't going to let certain characters die.
I don’t remember the exact timeline but I think it could have been salvaged as late as the start of season 7. Yeah there was plenty of dumb shit in 5 and 6, but nothing too egregious (yes even immortal Arya).
But those last two seasons was where all these long-running plot lines (Danny’s invasion, Jon’s parentage, the white walkers, stannis’ claim, Tyrion’s revenge, the prince that was promised, etc.) and they just… didn’t. Or if they did, it was fricking shit. A strong finish can make people even overlook the dumbest shit.

Make burning King's Landing ambiguous instead of a sudden out of character flip. The Lannister falsely surrender and kill her second dragon there (not because she just forgot their navy), so she burns the army that is half fighting and half legitimately surrendering, and maybe hits some wildfire from her father.

Pretty much get rid of all the terrible plot armor. Make the Long Night actually effect the world in a major way which makes rebuilding with a new system more believable. If you have to make Dany have a evil tyrant character turn do it earlier so we can actually spend time with that character instead of the 30seconds we get. Don't make Jon into a naive idiot unless you go down a cliche 'happy ending'

HBO tends be pretty hands off when it comes to their creators, and D&D had given them a huge hit so why not let them do their thing. Sometimes this kind of management works out great and you get shows like Deadwood, other times it goes terribly and you get John in Cincinnati.

Not to take the blame off of the Dabids, but I'm sure the main cast having been heavily involved with the show for more than a decade at that point probably made them want to be done with it so they could get on with their lives probably contributed to it.
I'm sure they all probably regret it now, but at the time the show had dominated their lives for so long the prospect of locking themselves in for another 5 years would seem miserable. They couldn't anticipate how badly the show's ending would land and what that would mean for their careers. In hindsight they took being stars in the biggest show in the world for granted, but in the moment they were probably exhausted.

There are many ways this could go (ie. first 3 episodes politics, last 3 episodes white walkers) but basically no simp Jon, no crazy Dany, no "innocent or otherwise" Jaime, no superhero Arya, no council that decides the new king, no peace after that council allows a region to split off from the throne, etc.

battle of the bastards was the high point of the show but even its season was bungled. there should of been other fights in the north leading up to the battle and entire episodes of gathering forces. also the whole Sansa hiding a massive relief force from Jon is when it really stopped making sense.
