I believe the SAG has like 10 pages of instructions on the safe handling of firearms for actors, which includes the instruction that it's the actor's job to finally ensure that the weapon is properly loaded. Alec himself used to claim he has extensive knowledge and experience with movie gunplay. It's Alec's fault, and no one else's.
