Court ordered mandatory stay in a mental hospital, but under a false name and with Bruce using some makeup or something to change his appearance so tabloids don't find out where he's at.

Exactly what sort of injury are we talking about here? Because walking around naked in a fake bender may warrant a meeting of Harley in an addiction ward, not make him bedridden for weeks. Bedridden makes more sense, since Bruce's crusade would never allow him to stay in inpatient if he was able bodied. Maybe something like a brutal car crash in a proto-batmobile would make more sense than a fight putting him in a body cast than losing a fight with people who would probably much sooner kill him.

Getting bruce in "in secret" wouldn't be that difficult either. Just have him get caught doing a batman thing, but have the time and presence of mind to change out of his costume. Since he doesn't have streetclothes underneath, maybe he gets caught running around naked in a park. He could spin it as a bender gone wrong or some other kind of rich and famous mistake and blame it on depression from his parents death to get out of an indecency charge. Set it when he's technically a minor, maybe like on his 18th birthday or something so the court records could be plausibly sealed. The timeline takes a little massaging for that since Batman stories generally depict him as starting in his twenties, but this is an AU.
Court ordered mandatory stay in a mental hospital, but under a false name and with Bruce using some makeup or something to change his appearance so tabloids don't find out where he's at.

I'd read it as a sort of companion piece to Harleen. The story of an under-appreciated medic (or in this case, nurse) with a less-than-enviable social and professional life meeting with someone who clearly isn't all there in the head. Only it has the potential to have a good influence on her (Bruce in this AU story) or it's a decidedly terrible influence on her (Joker in Harleen)
Exactly what sort of injury are we talking about here? Because walking around naked in a fake bender may warrant a meeting of Harley in an addiction ward, not make him bedridden for weeks. Bedridden makes more sense, since Bruce's crusade would never allow him to stay in inpatient if he was able bodied. Maybe something like a brutal car crash in a proto-batmobile would make more sense than a fight putting him in a body cast than losing a fight with people who would probably much sooner kill him.
I can’t think of a specific condition but I want this Bruce to look like Hush while he’s in the clinic. Basically can’t see much of him, so Harleen gets to know Batman rather than Bruce Wayne
You're not really going to get that kind of bandaging without his face being majorly fricked up. You could make it conceptual. Private clinic, anonymous treatment, Bruce isn't going to say who he is, Harley is forbidden to ask, and the whole thing is set before Bruce started his playboy himbo public persona.

Hell, could even have a few hints of what's to come with him harmlessly flirting a bit with Harley now and then, but that would probably be too OOC for Bruce given the circumstances, especially early on in his career.
Having him do a bit of flirting, even if accidentally, would be a good way to show his psychological changes he's undergone after becoming Batman compared to before and could also be used to hint that despite his assertions Bruce is actually a lonely man, which is why he surrounds himself with the Batfamily.
That's a pretty good way to see it, actually. And it'd help the wider narrative of Harleen falling for him, with her misconstruing his playful (potentially accidental) flirting for something more serious.
