The murder scenes are a power fantasy that exists in his head in which he is in total control exerting his will on others whereas in the real life scenes he's constantly sweating profusely and losing his shit while his friends barely pay attention to him or keep thinking he's beneath them except for Jean who wants to fix him and Luis who's just attracted to his toned body and fashion sense.
Also, remember the fact that Jean and the Luis are also his real life friends and he didn't kill them because he couldn't bring himself to do it for real.

There's also numerous parts where Patrick is clearly hallucinating. He talks about Evelyn looking like a giant, black ant. He talks about the Patty Winters show interviewing a Cheerio, and there's the park bench that follows him. It's also Paul Owen is actually alive, but it's also possible the dude who had dinner with him in London mixed Paul Owen up with another guy. It's never really clear what's real or fake.

Not the one with the girl, there was another project that was about Bateman eventually marrying Jean and having a kid and sorta trying to lead a "normal" life, but a series of emails from him show that he's growing more deranged everyday and he only keeps himself in check because of his son.
