Because mutations which survive don't involve extra chromosomes/missing chromosomes. The ones that survive involve the nucleotide sequence on a particular chromosome being changed. There's still 23 pairs, but one of those chromosomes now has a different string of nucleotides, which results in developmental abnormalities
