      He sense Frodo wearing the ring multiple times during his journey. He was very aware where was he heading. On top of that his orcs captured Frodo and Sam in the entrance of Mordor. So Sauron knew very well the presence of Frodo in Mordor.

            Osgiliath is halfway between the Black Gates and Minas Tirith, since then he had his armies wrecked by the good guys with Aragorn leading an army of indestructible ghosts

        up until Osgiliath as thats where a Nazgul even saw Frodo the last time and could have sensed the One Ring, afterwards he actually didnt wear it until he reached Mount Doom. Even when he lost the Ring it was Sam who took it but didnt actually use it either.

        the orcs at Cirith Ungol were more busy fighting each other over the Mithril cuirass than taking the hobbits to Sauron.

        All sauron could sense from someone putting on the ring was that it was being used somewhere. He could spot the ring bearer if he used it in certain spots like Amon Hen, but only because of the mystical power of that place acting like a beacon for a wizard like sauron. He would have zeroed in on Frodo when he used the ring there but Gandalf was able to distract him. The closer the ring bearer gets to mount doom the easier it is for sauron to sense the ring’s power but it doesn’t instantly give its position away like a gps

    Because the biggest threat in his mind was that someone like Gandalf, Galadriel, Saruman, or Aragorn could figure out how to use the ring and overthrow him with it, so his strategy was to be as aggressive as possible so that whoever found the ring wouldn’t have enough time to master it.

      Sauron had different fortifications in many those places during the second age but they were all razed to the ground during the last alliance. Even though Cirith Ungol was built by the numenoreans, the fact that it collapsed after the ring was destroyed implies that it was built on top of the foundation of an earlier fortress that sauron had constructed.

      Put a fence, dig a frickin giant trench with lava all around it, guard all the paths, put a traps all over the place, cealed all the entrances, put orc troops to patrol, having at least one Nazgul to 24/7 watchs the paths from above, have a wargs everywhere, put cave trolls at caves, put a giant Stone gate at the lava entrace, or frickin put a giant condom all over the Mountain.
