Incredible Hulk was the second film in the MCU, so they weren't too concerned with replacing the actor in a major role at that point. Perhaps Bruce/Hulk was supposed to make a cameo or post-credits appearance prior to the first Avengers film, who knows.
Also Norton->Ruffalo wasn't even the first instance of this, since they went Howard->Cheadle between IM1 and IM2, all happening before Ruffalo's first appearance.
