Doctor stories often involve someone just coming in sick and dying, just because that's just something that happens and doctors deal with that shit all the time. There's tons and tons of stories about parents getting over losing a child from an accident or children losing their parents and having to deal with being adopted, etc. There's even family sitcom episodes that's just about the kid's goldfish or hamster dying.

I watched this movie BECAUSE I got spoiled that it will be a tragedy. I would not have watched it otherwise. And still it hit me out of nowhere because I expected that she would get some illness in the end and we would see him slowly come to terms with it while she puts on a brave face etc. But it happening in 2/3 of the movie off-screen took me by surprise and was really effective.
