They all woke up there because they were killed by Saito's projections, but Saito stayed asleep because the drug was still active and there was nothing happening in the dream to wake him.

both would wake you up but when they were too deep all the dream levels had to kick at the same time so when the car fell off it didn't work because the other dream levels weren't kicking then

Took me a long while to get dreams in dreams. Waking from one into another but thinking the 2nd one is real is a big mind rape. Altho I have found a certain awareness of ones body making it known you're awake. Just gotta remember it in a double dream. Nicotine and kratom boosted the craziness of dreams a lot. As a daily stoner, even weed makes it more intense at times, doesn't keep dreams away at low doses anymore. Thanks for reading my gay little blog post

The dream level sedative is designed to not impede inner ear function so the kicks are supposed to work through the sedative.
They don't need a kick on the plane, they just need the sedative to wear off there.
The car chase has enough of these to make you question the logic though.
